This service agreement is entered into this day _______, _______,______ Between MID-PACIFIC INDUSTRIES, INC., 1500 Case Place, Woodland, CA 95776 and (Company) ________________________________ (Billing address) ______________________________________________ _________________________________________________ MID-PACIFIC INDUSTRIES, INC. will provide Preventive maintenance services to the following vehicle; MAKE: ______________ MODEL: _________ SERIAL #: ___________ ENGINE MAKE ________ MODEL: ________ SERIAL #: ____________ Anticipated hours per month usage: ____________ On site address: _______________________________________________ Mid-Pacific Industries, Inc. will provide service to the vehicle as per the manufacturers recommended schedule. Mid-Pacific Industries, Inc. will include in the service at no additional cost to the customer all required filters, engine oil, transmission oil, gear oil, grease, shop supplies and disposal / environmental fees as required by the preventive maintenance schedule, as attached. In addition to the regularly scheduled service Mid-Pacific Industries, Inc. will inspect the unit at the time of service to determine the mechanical condition of the unit and make any recommendations for repairs. If the customer chooses to have the required repairs be completed by Mid-Pacific Industries, Inc. at that time Mid-Pacific Industries, Inc. will complete the repairs on a regular time and materials basis. These charges will be over and above the preventive maintenance service in effect at that time. If the customer requests Mid-Pacific Industries, Inc. to do any repair or service on site at any other time than that of the normal scheduled service, Mid-Pacific Industries, Inc. will provide such service on a regular time and material basis which will include travel time, mileage, and service rate in effect at the time. After normal working hours, which are Monday thru Friday eight AM thru five PM, for the first four hours and for the first eight hours on Saturday, work requested by the customer will be charged at one and one half times the regular rate. All other hours, Sundays and holiday work will be charged at two times the normal rate. Mid-Pacific Industries, Inc. mobile service is based on the portal to portal concept as is standard in the industry. The cost of the scheduled preventive maintenance service is based on a one-year average cost taking into account that some benchmark services are much more costly than others are. The total annual cost is divided into equal parts according to the frequency of service to spread the cost evenly over the course of the year. It should be noted that this agreement excludes the 50 hours service, but does include the 250, 500, and 1,000-hour service. The price quoted is based upon a one-year agreement and Mid-Pacific Industries, Inc. will hold the price valid for this agreement for one (1) year from the signing, with the exception of oil products. Oil product price increases in excess of 20% during the course of this agreement will require an appropriate adjustment, which will be made with advance notice to the customer. It shall be the customers' responsibility to notify and inform Mid-Pacific Industries, Inc. when the service is due. When the service is scheduled or the customer requests service the customer must surrender the unit to Mid-Pacific Industries, Inc. upon arrival of Mid-Pacifics' service truck on location to perform the required service. If the customer does not surrender the unit immediately, the customer hereby agrees to pay Mid-Pacific Industries, Inc. all stand by costs until the unit is available. The customer also agrees to pay all applicable charges including travel time and mileage, even if the unit is not made available to Mid-Pacific Industries, Inc.
